  1. EACH MEMBER PLEASE FILL OUT AN INDIVIDUAL PROXY FORM.
  2. PLACE THE PROXIES IN A SEALED ENVELOPE
  3. WRITE PROXY ON THE ENVELOPE
  4. SEND BACK TO THE SCHOOL BY MONDAY PREFERABLY AM.


 

  1. IN YOUR CHILDS FOLDER
  2. IN THE PICK UP LINE
  3. IN THE DROP OFF LINE
  4. DELIVERED IN PERSON TO THE OFFICE

    


 

ALL PROXIES WILL BE COLLECTED BY TEACHERS / STAFF / VOLUNTEERS AND HAND DELIVERED TO THE PTO BOX IN THE OFFICE.
